Primary Milia
What really is Primary Milia?
We have all had that pesky bumps on our face and sometimes on the arms, that just refuse to go. Sometimes they look like small pimples but once you pop them you find they are something very different.
Popping usually results in a lot of gunk and scarring. Little do you realize that most of the time, this is the typical presentation of primary milia?
So, what is milia and why is it so problematic for you?
Basically, milium is a small gunk filled cavity called a cyst. They are usually harmless and appear like white pearl-like bumps on your skin. Although primary milia hardly cause any harm, the scarring and the pigmentation that results from popping them can be very disfiguring. Here we tell you all about milia and the different types. Keratin encapsulated by the skin, usually with primary milia, a reaction or interaction with your Vellus hairs.
So, what is Milia in Adults?
Basically, these are cavities lined by small cells containing keratin. They are usually found in groups around the nose and the eyes or on the cheeks. They are formed due to excess keratin becoming trapped beneath the skin. As the keratin cannot escape, it raises the skin forming a white/silvery bump usually called Primary milia. You might wonder what keratin is.
Well, keratin is a structural protein that is found in skin cells, hairs and nails. Normally, a check and balance are kept o the production of this protein. However, when there is excess deposition of keratin, it causes the formation of milia, or a singular Milium.
Where does Primary Milia occur?
Milia can potentially occur on any part of the body. However, it typically occurs around the eyes and nose. This may be because of the creases and the excess formation and sloughing off of cells in these area,s that leads to these silvery dots scattering around the face. Milia can also occupy the eyelids and the cheeks.
Which people get primary milia?
Milia can involve people of any age group. The condition affects both males and females equally. However, there are different kinds of milia that can have a predisposition for different age groups. Primary milia usually occur in children. However, it can affect a lot of adults as well.
What causes Primary Milia?
Basically, the precise cause of milia is still not found. Especially milia in newborns is idiopathic and doctors have failed to find the reason behind them. the baby milia is seen as a connected skin condition with the Vellus hair shed from a new born.The condition is also often misdiagnosed as baby acne which can lead to harmful chemicals being applied to the baby’s skin in order to fix it. Please never do this as it will open up a can of worms of other skin conditions for your child.
The basic difference between primary milia and actual acne is that milia does not cause inflammation of the skin. Another difference, according to experts states that babies born with primary milia have these spots at birth. However, baby acne occurs a few weeks later.
Clinical Features of Primary Milia
Primary Milia may occur without associated symptoms. However, often primary milia is associated with some type of damage to the skin. This usually occurs in older children and adults. The condition is caused due to too many dead cells becoming trapped in your skin and obstructing sweat ducts.
Primary milia often accompany skin reactions or are due to irritation from harsh chemical products.
Sometimes milia can also occur due to excessive use of facial creams that hinder the natural exfoliate effect of the skin. However, usually, primary milia is caused due to unknown reasons which make its regular treatment difficult.
Commonly associated Milia symptoms include:
- Blistering skin due to the associated condition
- Burns
- Blistering skin due to poisonous toxins like that of poison ivy.
- Derma-brasion and laser resurfacing and other skin resurfacing procedures.
- Steroid creams that lower immunity and clearance of dead material from the skin.
- Prolonged damage from the sun
Types of Primary Milia
Types of Primary Milia
Milia occur in a variety of forms and shapes. They are small, dome-shaped bumps that are usually white or yellowish. They may be not itchy or agonizing. However, they could cause discomfort for a lot of reasons. Rough mattress sheets or clothing can cause milia to become annoying and red.
There are many types of milia. These cysts are categorized based on the age of which they arise or the incident that triggers the cysts to build up. All Primary Milia versions arise from issues with the Vellus hair connections.
What are versions of Primary Milia Bumps?
The various classifications of Primary Milia are in no particular order, as follows;
- Primary Milia.
- Multiple eruptive Milia
- Genodermatoses-associated Milia
- Milia en plaque
- Congenital Milia
- Nevus de-pigmentosus with Milia
- Benign primary Milia of adults and children
- Nodular grouped Milia
So lets break down the varying aspects of primary milia, into its distinct classifications;
Multiple eruptive
Milia Multiple eruptive Milia is as its name suggests, a case of milia which out of the blue, erupts in patches on your skin. To get to their full size can take as long as 3 months or as short as two weeks.
Their skin position is usually a lot more irregular, as is their shape. Random edges on the Milium ‘cap’, are a good example of this skin condition. Broken down into three categories, chromosal genordermatosis, single gene genordermatosis or polygenetic genordermatosis are usually the culprit. This inherited skin disease is very common with this DNA traced auto immune irregularity.
This form of milia has also been closely tied to such genordermatosis disorders such as; Orofacodigital syndrome I,Gardners syndromeBasaloid follicular harmatoma syndromeBazex syndrome, Saying that, as medical history has proven before, you can have a large outbreak of Multiple eruptive Milia for no genetic known reason and you are just suffering from an autoimmune ‘glitch’.
Genodermatoses-associated Milia
Genodermatoses milia is classed as an auto immune disorder. The genetic makeup of the underlying disorders, produces the milia seeds as a side effect. This form of milia are common around the edges of other skin conditions, compacting the issues with the bad skin. This also compounds the treatment spectrum, due to association with other conditions that could be averse to the treatments.
This form of milia, in the medical profession can be seen as sign of underlying, more severe medical conditions. This presentation of Milia can be associated with a wide variety of auto immune disorders, such as ;
- Rombo Syndrome,
- Atrichia with papular lesions,
- Rookespielger syndrome
- Basal cell nevus syndrome,
- And pachyonychia congenita type 2.
Milia appears with a very high percentage of cases of Bazex-Dupre-Christol syndrome.
Milia en plaque
Being one of the rarer forms of Milia infection, this form of milia can easily be Classified. On a, sometimes large plaques around the 5cms size range, they are a large group of milia. The 1-3mm pearls have joined together forming a solid state plaque that is formed of multiples of individual Milia engorged cells.
They are very common around the ears or actually to be more accurate, behind the ears. Due to their specific look and texture they can be mis-diagnosed as Favre-Racouchot disease or Come-done nevus. Make sure you get an expert opinion on what the plaques constitute. The issue lies, that unlike most other milia infestations, this form of milia can have no raised areas at all and is based in the dermis only or even the sub cutaneous fat layers.
They can just manifest for no reason at all, but they have been associated with the auto immune disorders as follows;
- Pseudoxanthoma elasticum.
- Discoid lupus erythematosus
- And Lichen Planus,
Due to the irregularity in the thickness and position of the Milia plaque, it can hinder treatment, sometimes taking slightly more time for the treatment to break down the Keratin interlocking, before successful removal can be achieved with Cerobio Milia Removal Cream
Congenital Milia
With a very high percentage of new born babies, being born with neonatal Milia, or as it is medically known, Congenital Milia, it’s a very common skin disorder with babies. Some babies don’t have them on birth but they can appear over the next couple of days to a week. The main thing here is not to worry.
As a possible new parent who is reading this fretting over those white bumps on your new precious face, relax, most of the time they will disappear. 95% of babies born with new-born milia are clear if it within the first month.
When it arrives in juveniles, it is still classified as being Congenital Milia. It is a common result of underlying medical disorders, such as;
- Christol syndrome
- Pachyonychia congenital
- Gardner syndrome
- Nevoid basal cell carcinoma syndrome
Although it can appear for noreason Medical opinion on all aspects of Congenital milia is that it is an issue with the Vellus hairs, as they go through their shedding cycle.
Nevus de-pigmentosus with Milia
The occurrence of Neuvus de-pegmentosus Milia appears in places where the skin has no pigment. Hypo-pigmentation is a skin condition in its own right, but can also be compounded by the appearance of Milia.
This particular form of milia, will not go out further than the de-pigmented skin.
Seated in the papillary dermis, they are small epidermal cysts formed in the basal layer hypo-pigmentation. Whilst the issue with the skin is the lack of pigmentation, you can have this form of milia with a normal number of melanocytes.
This is also one of the kinds of Milia that can be either primary Milia or secondary milia.There are lot of reason for the hypo-pigmentation of skin, from radiation, to auto immune disorders. Luckily this is a rare occurrence for the milia to be embedded in the hypo-pigmentation.
Benign primary Milia of adults and children
Benign primary Milia, very commonly, just appears for no reason. Appearing on all aspects of the general population, it can be found on the face, lips, ears and even the genital areas.
The school of thought as it is so random in its appearance, it that it is a glitch in the bodies system, which produces either a temporary auto-immune flare-up, or is the manifestation of very subtle borderline auto immune irregularities.It is also a very common after effect of too much sun.
Sun tanning can induce it, this is why there is a second school of thought that this is the result of external factors and is induced by the skin soaking in harsh elements that can affect the cells well being.
Seen as this is so a regular form of milia, it’s understandable that is one of the common culprits is the damage induced by UV rays, then this is great excuse to keep that skin protected from the elements.
Nodular grouped Milia
This form of Milia is when the keratin is surrounded by liquid. Very irregular in nature, they are basically the same as standard milia, but with a moderate lymphocytic inflammatory infiltrate encompassing them. They do by nature cluster together and can expand into each other to form larger collectives.This is the generic version of Milia, when there is no longer a classification on the milia structure. Basically, once the milia has been engorged with the liquid, it fits into this category.
This is another one of those Milia, where they can appear for no apparent reason. An external factor, sun damage or bad skin cleaning routines can all have impacts on your skin and on this basis, this form of Milia can result.
Just to make sure of the clarification on the over all difference with Secondary and Primary Milia is that Secondary Milia has eccrine attachments and primary is classified when there is a cellular attachment to the Vellus Hair.
